Working with Case Managers

Case management support
We work alongside case managers to turn clinical recommendations into installed, regulation compliant adaptations that keep rehabilitation on track.
Our role is to plan, coordinate, and install to the drawings and specifications set by your appointed professionals.
You receive clear options with risks, costs, and timelines, then a scoped route that supports safety, independence, and carer efficiency.
Works are sequenced around therapy schedules and care arrangements to keep disruption low.
Turning clinical recommendations into installed outcomes
There is often a gap between what an Occupational Therapy report requires and what a general contractor understands. We close that gap on the installation side.
We review the clinical aims with the professionals you appoint, check buildability and services, and deliver an installation that supports the intended functional outcomes.
The goal is a home that works for rehabilitation and daily living without drifting from the clinical brief.
Our process ensures that the home environment is meticulously re-engineered (exactly to spec) to become a high-performance setting for rehabilitation, directly supporting your client’s long-term Activities of Daily Living (ADL) goals.
Governance, risk, and financial transparency
Budgets are itemised and traceable. Any variation needs written approval with a short rationale.
Risk registers, safeguarding measures, and RAMS are agreed before works begin.
At handover you receive a complete compliance pack with certificates, warranties, manuals, and photo evidence, so outcomes can be demonstrated to funders and legal teams.
The administrative trail is designed to withstand legal and financial scrutiny and to protect your position in complex matters.
Assurance you can file with confidence
- Fixed price itemisation that prevents scope creep without written approval
- Digital handovers with O and M manuals, warranties, and relevant NICEIC and Gas Safe certificates
- Site specific RAMS suitable for neuro rehabilitation and complex care environments
Working with your multidisciplinary team (MDT)
Successful outcomes depend on simple, consistent communication. We integrate early with the OT, the Deputy, and the family. Assessments are reviewed, functional targets and constraints are confirmed, and a practical sequence is agreed.
Technical drawings and schedules from your architect or surveyor remain the reference point. We provide plain language summaries of what the installation will achieve, plus costed alternatives where there is more than one viable route.
Site activity is planned around therapy sessions and care rotas, and progress is shared through concise updates at an agreed cadence.
What you can expect in practice
- Early review of assessments to flag constraints that could hinder therapy later
- Plain language summaries that link technical works to functional outcomes for faster sign off
- Itemised alternatives that help you justify spend to deputies and insurers
- Sequencing that protects therapy continuity and safe access
Outcomes aligned to rehabilitation goals
Progress is measured in real life gains. Safer transfers. More independent activities of daily living. Less fatigue. Smoother carer workflows. When speed matters, we stage delivery so essential function arrives first.
Temporary measures support safe discharge and everyday routines. Permanent works follow when funding or tenure is confirmed. This phasing keeps the environment in step with therapy milestones and reviews.
Scope of works we install to your appointed designs
Every instruction is different. Some require rapid, reversible measures. Others involve significant change coordinated by your design team. We install the technical infrastructure required for complex equipment and safe access.
- Structural alterations aligned to approved drawings where load paths and clearances are defined
- Specialist hygiene environments such as level access wet rooms with appropriate drainage and ventilation
- Access and egress improvements including automated doors, ramps, landings, and threshold levelling
- Rapid discharge measures that deliver immediate function in permanent or interim homes
- Early stage Specialist property assessments and consultancy support when feasibility needs testing before purchase
Our motivation
An adaptation is more than a construction task. It is a way to restore dignity and reduce environmental press so the client can do more with less help.
Delayed discharge or a poorly positioned hoist is not just a cost line. It is a lost week of progress. Our site teams work with empathy as well as technical care.
Safeguarding and emotional intelligence are part of the brief. We are guests in a home during a vulnerable time and we act accordingly.
We keep the focus on the why. Installing a wash dry toilet can reduce intimate care. Widening a doorway can reunite a parent and child at bedtime.
These practical changes matter because they translate directly into daily wins for the client and the family.
